The road has been posted as a US highway since 1930. From 1930 to 1934, it was US 122; in 1935, that designation was given instead to the Pottsville-Reading road in Pennsylvania (now PA 61) and this road was labeled US 202, for its status as a spur off US 2 at Bangor, Me. The southern terminus of US 202 is US 13 at New Castle.
However, in Bergen County, US 202 is not under state jurisdiction. Instead, it is maintained by the county; all but the northernmost 0.04 mile is designated County Road 91 (the remainder is part of CR 507).
A short 0.32-mile section of the road, between Yawpo and Franklin Avenues in Oakland, is state-maintained thanks to the project that upgraded NJ 208 to I-287 in 1993.
